Online Forms are provided for your convenience, and some can be used in coordination with an online payment portal.  If the form name is preceded by "$", it can be used with online payment.  Please follow the steps below to submit a form and pay online:

  • Select the PDF fillable form for the service you are requesting.  Download and save the form on your device, prior to filling it out.  Fill out the form and re-save.  If you are unsure of which form you need or how to fill out the form, please contact us at (330) 493-9904.
  • Upon completion send the filled-out form via e-mail to [email protected].  Also, send any necessary attachments with the form, such as drawings in the case of an additional evaluation.  (The form will state if an attachment is necessary.)  
  • The form will be reviewed by our staff and an invoice generated for the service to be performed.  You will receive an e-mail back from our office with the invoice, invoice number, and a hyperlink to complete your payment.
  • Once you have made payment the form will be forwarded to the appropriate staff member to complete the requested service.  Please note that online submittals are processed in the order they are received along with those made in person.  It does not expedite the scheduling process.
