ADA Accommodation Request Form

The Stark County Family Court will provide reasonable accommodations for anyone with a disability, enabling individuals to participate in Court services, programs, and events provided that the accommodation does not fundamentally alter the nature of the hearing, program, service, or event, or impose undue hardship on the Court. While the Court may not be able to fulfill your exact accommodation request, the Court will strive to provide an equal opportunity to participate in all Court services.

To request an accommodation, complete and submit the Court's Title II ADA Accommodation Request Form at least seven (7) days prior to your hearing or event. The form can be completed and submitted below.

Stark County Family Court will not share your personal information, material, or documents other than for purposes of business necessity.
